About Our Members:

ISHA membership is made up of distinguished clinicians who have distinguished themselves through advanced education, and the clinical care of patients. The membership is an eclectic group of clinicians that focus on the care and maintenance of the stomatognathic system.

How to Become a Member:

You may submit a membership application through our secured online server, or you may download the application in PDF format, and mail it via USPS to:

New member applicants should include a letter of nomination from an existing member with their application. Final membership approval will be made by the general membership.

When completing the online application, please create a user name and password in the first section of the application. 

Membership Fees:

Initial Membership Fee: $1,200.00

Annual Membership Fee: $385.00

Initial membership fee as well as first year's annual fee must be submitted with the initial application.  If the membership application is not accepted, all fees will be refunded.
